I see two or three patients a week who have had injections of something into their buttocks. Many times they are unsure of the product. Silicone may pose an unnecessary risk to fat transfer in this situation. In the past, I have cut these areas out and then fat grafted at a later date. If the silicone has been injected in the distant past without difficulty, then fat transfer may be appropriate. However, the patient should understand that there is probably an increased risk of problems (infection, wound healing, fat necrosis) following the surgery.
This is the type of problem that really requires an in person consultation to give you the appropriate advice. There are all different types of products, some of which have had disastrous effects. It may require 2 surgeries - one to remove the product and one to reconstruct with fat grafting, but without seeing you it would be impossible to tell. Good luck

I usually require a hemoglobin of 12 to undergo a BBL since so much liposuction and therefore blood loss occurs with this case. The goal is to avoid having to give a blood transfusion. Taking iron pills for a few weeks to one month should get you up to 12.
I do not use drains. Most of my out of town patients stay in town from 3 to 7 days depending upon multiple factors. Absorbable sutures can be used. Sometimes these sutures do not dissolve as quickly as necessary, and they must be removed. I would ask your plastic surgeon what should be done.
