What percent of buccal fat should be removed?

I had an appointment with a plastic surgeon who recommended me a buccal fat removal for styling my round face but I have the question of how much has to take off because I've been reading that over time you're losing that fat and you can get an older image. Would it be better to have a subtle result now so that eventually it is not excessive and bad looking?

The right amount of buccal fat to be removed should be measured in accordance with the patient's anatomy and wishes. The amount of buccal fat removed during the procedure depends on multiple factors. Some of the factors are chubbiness of the face, facial structure, and patient's goals. In my practice I tend to remove not all but some of the buccal fat (approximately 70 to 80%) to have a nice pleasant result. In patients with significantly large chubby cheeks or patients that want to achieve a model like hollowed cheek look, then the entire buccal fat is removed. Patients won't look aged after buccal fat removal if their cheek bones are prominent.

Generally speaking, if you are a good candidate for the buccal fat procedure and have excess fat in the mid-cheek area the surgeon removes all the buccal fat.  The results from this procedure are usually very modest, not extreme at all.  I have never had a patient complain that they see too much of a result, that their face is too thin or angled. But I have had numerous patients tell me they see the improved result but they wish it was even more noticeable. This is why I in most cases I remove the entire fat pad. Make sure you consult with a board-certified plastic surgeon with experience in this procedure as there are some tricks to it that make it a very safe and predictable procedure.
