What would be the best surgical plan to achieve results? I have attached 2 pictures. The picture on the left is my current nose. The picture on the right is a picture of when my nose was swollen after my primary and I was very happy - so the picture on the right is my desired result.

Because you don't mention when your surgery was done, let me first suggest that you wait about 1 year for a revision. The nose can be lengthened and bulk added to the tip; this is not a simple procedure - select a surgeon who specializes in revision rhinoplasty work.
