I am a 43 year old female. My problem areas are a deep wrinkle around my mouth, my nasolabial folds, and the skin from my chin to my neck which has started to sag. I'm happy with my upper face so I thought a mini facelift would be suitable? or would something less invasive work as well? I have tried check fillers which made no difference, and RF therapy, which did absolutely nothing.

Answer: Midface lift with neck lift Based off your photos without addition angles and view so the entire face to fully understand your facial anatomy - looks like deepened NL folds asymmetrical along with some looseness / laxity of your anterior neck with a few deepened folds there as well. There are actually a few options to tackle these areas and different levels of surgery with different levels of risks - but I think surgical intervention now is needed since you have tried all the non-invasive procedures already. Likely you will need to get your fillers dissolved prior to anything surgical and make sure to let your surgeon know everything you've had done and where. Midface lift / rejuvenation - this would address your cheek bones and the NL folds; fat grafting can be added to the procedure as a combination to really move all your facial anatomy more vertical back to where it was when you were younger (an analysis of your eyes and brows would help to also see if a brow lift could also add more youthfulness back to your face) This can be done through an incision hidden in your temporal hairline. Neck rejuvenation - can try some lipo and skin tightening versus a neck lift (incision behind the ears or submental incision) The first has lesser incisions but also a lesser guarantee if the skin will be as tight and clean as the second option. CO2 laser can also be added to any area where you have fine wrinkles or under eyes, where skin is just a tiny bit loose.
