If you are trying to add more volume, I would suggest waiting about 6 months. If you are needing fat to help improve the inner breast area or hide rippling, then fat can be added around 3 to 4 months.
Grafting fat over implants is a little tricky and different plastic surgeons will have different opinions about it.One issue to take into consideration is the possibility of the cannula injuring or rupture in the implants.For that reason some plastic surgeons myself included I will remove the implants during fat grafting and then put them back in the pocket after grafting is complete.The space where the implants it can be temporary filter using a temporary breast implant sizer which is a disposable device used for determining implant size. Unfortunately, there is nothing you can do at this point but wait out the final result which typically takes at least 6 months. At 1 month, your breasts are still in the most intense phase of managing swelling, inflammation, and fat graft evolution.
Once that fat has completely healed at about 6 months, then mammograms and other activity should not be a concern, and your added fat will act like the natural fat in your breast.
The amount of fat that can be safely removed is based on the amount of fat present and how many areas will be treated and your overall health and age. Your surgeon should be able to help guide you in this. Going over 5 liters of fat will require overnight IV fluids and observation.
