Protruding mouth and saggy cheeks. What would fix this? (Photo)

Im 19 and ive noticed sence grade school my mouth area sticks out and my midcheeks are super flat, it looks like my cheeks are on my jaw. I dont know what would fix this. I haven’t met anyone who has the same problem
