I am 5’8, 157 pounds and 32 years old. I have volume loss 2/2 weight loss. I am not a mother however I do plan to have children in approx 3-5 yrs. I was recommended 450 cc implants to achieve the desired lift effect as my chest is a 13.5 diameter. I was also recommended that a crescent lift to my right breast was done and an implantable mesh bra. I understand the mesh is supposed to help with “bottoming out”. Is it necessary? Should I just do a lift? Or wait until after kids altogether.
Answer: BA Questions Good afternoon, Do the crescent for symmetry, but skip the mesh. I perform over 500 augmentations a year and have been doing so for 21 years- first timers don't need mesh, they just need proper implant placement. Maybe your surgeon has a track record of causing bottoming out in his or her patients (?) or doesn't really understand what the mesh is meant to be used for (?) or is trying to upsell you on the procedure (???).

Answer: BA or BA with a lift I honestly would suggest waiting until after you had children unless thus is something that bothers you right now. If you are interested in getting the procedure now I would recommend getting a breast augmentation with a lift!

Answer: Long term If you are wanting the best overall long term result then a lift with implants is the correct choice. You can proceed with an augmentation only at this time, but you will likely need a lift in the future. Your breasts sit low on the chest wall and the weight of the implants over time will likely result in some sagging.

Answer: Implants I would need measurements to be sure, but you might consider just implants for now and no mesh and wait until after having the all the children you want ton see if you need a lift. Yes, you would still have a small difference in your nipples, then this would not be noticeable in a bra.

Answer: Breast lift and small implants or fat transfers A better long term procedure would be a breast lift. Your breasts are too low on the chest wall and adding the weight of implants will result in more descent of your breasts. I recommend a breast lift and small implants placed totally submuscular or fat transfers. Best Wishes, Gary Horndeski, M.D.

It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.
