My upper lip is 18mm My chin is only 25mm Would lengthening my chin by 5mm (via geniplasty) be enough? I feel my current doctor is being too conservative and that I need at least 10mm lengthening, giving a 2:1 chin:upper lip ratio

I recommend jaw surgery when there is a functional deformity (difficulty eating/chewing food). Otherwise, a sliding genioplasty or chin implant. With small horizontal movements, a chin implant is easiest.
It appears that both genioplasty and chin augmentation with implant would be viable options in your case. In person consultation with a board certified plastic surgeon is recommended.
