So when there is volume loss there, not only do the cheeks reflect this, but the under eye can as well with the development of tear trough and under eye hollows. The eye area needs significant combinations to get improvement. Lasers like erbium, co2, halo, profractional/contour, or Fraxel can all be used to build collagen long term. The upper cheek region is a very important area for contrbuting to a more youthful appearance to the under eyes. When there is signficiant volume deficit in this region, the area is flattened and the under eye regions can appear hollow with tear trough development--as seems to be the case here. If this is indeed so, then the use of a robust HA volumizing filler appropriately injected into the upper cheek region (more medially in order to maintain a masculine face), not only will the cheeks appear more youthful, but the under eye region will secondarily be improved, This can be accomplished by quite quickly in about ten minutes in the hands of an experienced, physician injector. Your problem is not really aging but a combination of congenital volume deficit and probably residua of the osteotomy. The easiest treatment is use of a heavy filler like Juvederm Voluma deep. Be very careful, however, who you choose to do this as most injectors use a light filler superficially which can lead to significant problems.
