Part of what defines your waist to hip ratio is the anatomy of your rib cage, your abdominal wall, and your pelvic bones. Some modification of the abdominal wall is possible with tummy tuck surgery to tighten the muscle layer and reduce the cylinder of the abdomen a bit; most of the time, nothing can safely be done to alter the bony structure of the ribs and pelvis (some patients have had rib removals of the lower ribs to try to narrow the waist but I consider this extreme and not necessarily a safe procedure). Because some of the definition of the ratio is based on how much soft tissue is between the fixed structures of the body wall and the skin (i.e., the fat under the skin), liposuction body contouring procedures can often help improve the waist and narrow its appearance.

Having a curvy waist line is very attractive and feminine. If you have protuberant flanks/love handles due to unwanted fat deposits, resistance to diet and exercise, liposuction/liposelection is the answer.
