I find that properly formed liposuction can help improve this. I recommend VASER with ThermiRF to remove the fat, lift the skin, and contour the jawline. I pretreat everyone with Botox 1-2 weeks before surgery to relax the neck muscles, allowing for better retraction of the skin after surgery. A consultation with a board certified plastic surgeon would be helpful to evaluate the amount of fat around the neck and what treatment modality would be optimal. Liposuction would be a reasonable option, but there are also non-surgical options such as Kybella injections. Given your age and appearance of skin elasticity I would expect there to be good retraction of overlying skin after surgical or non-surgical options.
The problem is attachment of the skin to the platysma muscle and fat deposition between the attachments. The solution is to lower the hills and raise the valleys i.e. liposuction of the rolls, release the attachments and fat graft as needed into the creases. You may benefit from removing fat in the bulging areas and "lifting" or reducing the depth of the creases. Liposuction can reduce the bulges to soften the transitions on either side of the creases. The creases themselves may benefit from something called "subcision" where a sharp tool is used under the skin the break apart the attachments to the skin and create a space in which some fat could be grafted. Even without the subcision and grafting, liposuction could be very beneficial alone in reducing the prominence of the rolls.
It is difficult to diagnose the issue at hand from your photos and I would encourage you to consult with a board certified plastic surgeon to discuss options. Many options may be considered, such as liposuction and various technologies/devises to treat focal areas of fat. A complete examination is needed to determine the location and extent of the fat beneath your skin and the presence of attachments to the overlying skin to determine the best course of action.
Thank you for your photos. It is best to have an in-office evaluation to determine what factors are contributing to the "rolls" It may be a combination of fat, loose skin, and/or genetics. Creases or skin folds on a body are often associated with skin attachments.You probably have a moderate amount of fat below your skin.Liposuction would most likely help though some of the folds may still be present but would be reduced in size.This is previously been stated an in person evaluation would be needed to make a more accurate assessment.Best,Mats Hagstrom M.D.
To obtain the best opinion, you should see a board certified plastic surgeon in person. It's unlikely at your age to need skin removed, but you could probably have liposuction and may be a candidate for the cool mini (a Coolscupting or fat freezing treatment) or kybella treatments, which is an injection which helps dissolve localized fat.
Hi,Thanks for your question. It is certainly possible he will be able to see evidence of your liposuction. Follow scar care recommendations of your plastic surgeon to minimize scarring. You may still have bruising, swelling and tenderness.
If you have any infection that would preclude you to have elective surgery in my opinion. I would repeat blood tests and speak to the dental professional to see exactly the type of dental issues you are having. Why have you NOT told our chosen surgeon yet??
