Does a nose with lots of graft look fake in real life?

Thank you for your question. Revision rhinoplasty is a challenging case and requires fine workmanship, extensive experience and deep knowledge of a whole anatomy. In right hands the grafts look natural and you won't have an overall operated nose appearance. 
You may achieve a beautiful and natural looking nose that suits your face by the Closed and Atraumatic Rhinoplasty. There is no cut in Closed rhinoplasty technique and the whole operation is performed within the nose. Since there is no cut on the nose, there are no scars or stitches. With closed technique, the nerve, ligament, vascular tissue at nose tip are protected so that the blood vessels and nose's flexibility ability is not deteriorated. There is nothing wrong with using cartilage grafts, but their use, especially in revision rhinoplasty can be very tricky.  Make sure you go to a plastic surgeon with plenty of experience in revision  rhinoplasty to give your the best outcome.

Revision rhinoplasty will not look fake if done by an experienced revision rhinoplasty surgeon.  see several experienced revision rhinoplasty surgeons in consultation and this will give you an idea of the results.

When good technique is used, cartilage grafts in revision rhinoplasty can have a very natural looking appearance.   Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system.  You should be able to view numerous pictures of actual patients with noses similar to yours performed by your surgeon.  If you cannot find enough real before and after pictures, then consider continuing your search for a surgeon.

Adding grafts alone do not make a nose look fake. A nose looks fake when it appears unnatural. This means the shape and size is not appropriately designed to harmonize with the rest of your face. Grafts are simply a tool to control the shape and size. It is up to surgeon to make sure the grafts create a natural appearance rather than a noticeably "fake" one. Hope this helps!Johnson C.  Cartilage grafts  arevonly used when necessary to build up a deficient area in the nose. Some revision rhinoplasty procedures do not require any cartilage grafting. Revsion rhinopasty is more difficult than a primary rhinoplasty due to the fact that there  has been previous alterations to the normal anatomy, and scar tissue present. An in-person examination is going to be required to make a determination about how best to proceed. Its also important to know how much cartilage is left over on the inside of the nose for potential rafting purposes, if needed.
