I was punched in the face at 14 and my nose was slightly broken but not fully but it seems to have healed crooked as well as with a bump. Is the crookedness fixable? I know the bump is but can it all be done without changing my nose drastically? And also what can be done about a deposit of fluid or fat under the chin from thyroid issues? Anything?

Answer: My nose was broken when I was younger and seems to have healed crooked, can I fix the crookedness or is it too late The areas that you described and would like improved can be addressed with a subtle natural appearing rhinoplasty performed by an expert artistic plastic surgeon with extensive rhinoplasty experience. Keep in mind, that following the advice from a surgeon on this or any other website who proposes to tell you exactly what to do based on limited 2 dimensional photos without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of the proposed operative procedure with you and your parents may not be in your best interest. I would suggest you find a plastic surgeon certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) or facial plastic surgeon (otolaryngologist) that you trust and are comfortable with. Rely on the surgeons before and after photographs of his actual work and not imaging. How the surgeon does this is not as important as what he actually can produce.

Answer: Rhinoplasty for crooked nose and dorsal hump The dorsal hump is composed of both bone and cartilage and must be shaved down in order to remove it. Osteotomies placed in the nasal bones are required to straighten them. When there is a crooked upper lateral cartilages in the midportion of the nose, a spreader graft is used straighten that area. My recommendations would include reducing the hump, narrowing the bony base, repostioning and narrowing the tip and alar rim grafts to lower the nostrils. You certainly should have an in-person consultation with a board-certified plastic surgeon or a facial plastic surgeon well-versed in rhinoplasty. Chin is hard to comment on without a physical exam but if there are areas of asymmetry that can be corrected with a combination of gentle liposuction and fillers or fat grafts. Best.
