It seems you have a crease that is obscured by hooding of skin.  If a standard skin only blepharoplasty is done you may have a nice crease show without needing to alter your crease further.

You may want to talk to a doctor to see if fillers may work for you. Maybe talk to them about Belotero, or what other fillers they recommend. this is something that you do not need surgery for unless it is necessary

I agree with the other reponses, but the one thing I will add is to make sure the compress are "warm" and not "hot". I see many patients who come in and say they've been trying to get the compresses as hot as possible prior to using on the eyelids and this is not a good idea. "Hotter" is not...
