Without actually seeing and feeling your nose in person, it is hard to say what the "lump" is. However, my guess is that it is cartilage that has been re-configured during your surgery. If steroid was injected to reduce what was felt to be scar tissue, it most likely just thinned out the area surrounding the "lump' and probably made it more obvious. It sounds as though you will probably need an exploration to correct the "lump", fill in the depression and improve the tip & supra tip areas. If you were just interested in improving the indentation without anything else, you might try having the doctor wash away the depot steroid with injections of saline. I have done this on several patients who have come to me after steroid injections and it usually takes about 3 or more at 1-2 week intervals.
