My skin isn't shrink wrapping at sides of my nose, making it look fat & wide. Will rib cartilage help give definition?

my surgeon who did my primary would like to use rib cartilage for the columella strut as well to increase definition as my skin is so thick. Can This work? I also want decreased tip width from the front and nostril reduction
