Bleeding suture following breast augmentation and uplift, 3 weeks post op. What can I do? Shall I cut it? PS is abroad. (photo)

My dissolvable stitch must have been poking out during my shower and got caught and I started bleeding. I’ve been bleeding for 3 days non-stop. My PS said it doesn’t seem to be infected from the pictures but I have a fever now and the right breast is hard, stiff and heavy. Is this a cause for concern? What can I do? The stitch is still poking out, shall I cut it? I had my surgery done abroad so can’t just go in to see my PS :(
