Hi! You look like a great a candidate for the ICON 1540 fractional laser! It helps with acne scarring and larger sized pores. The laser will help soften scar tissue over time and improve skin texture. These are best when done in a series of 3 or 5 depending on the severity of the skin. These treatments should be alternated every two weeks with a medical grade facial called a diamond glow. The facial will exfoliate the skin leaving it smoother. The solution used should be a brightening one such as tranexamic acid, which will help break up melanin and hyperpigmentation. You will need a few days of down time in case of swelling and redness which is normal.
Fractional laser can help the quality and erythema of the skin. It is an ablative laser that will remove the upper portion of the dermis and epidermis allowing it to remodel. The results will be long lasting in terms of the texture but your skin will show normal signs of aging after another 5-10 years. This procedure also has a relatively long downtime of 6-8 weeks of erythema. For those texture problems typical of acne scarring, Microneedling would be a better first line treatment.
Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Dark spots can improve with these treatments. I recommend fractional laser for incisional scars as well, especially after plastic surgery. Best,Dr. Karamanoukian Realself100 Surgeon

While you could attempt to improve the texture with multiple CO2 laser treatments, a scar that is that raised in that area would likely benefit from re-excision and injection of steroid. After the texture is improved, if the erythema remains, can try a PDL laser to decrease the redness.

In general larger procedure with extended wound healing are not recommended following radiation. However, you can do a small test area in or around the area affected by radiation and see how this heals. You will have a higher risk of these areas healing either hyperpigmented or hypopigmented...
This is quite normal and correctable. You may want to use Melarase PM cream and Exfolase to reduce pigmentation followed by RF Fractional for your stretch marks. Best, DR. Karamanoukian Realself100 Surgeon
Fractional laser with CO2 has variable depth. Avoidance of sun and application of a sunscreen and skin lightener is a must. In our office, we use Spectrase SPF and Melarase AM and PM after fractional laser. Best, Dr. Karamanoukian Realself100 Surgeon
