As a general rule, I do not prescribe antibiotics either before, or after lipoma removal.  In my practice the procedure is performed in an on-site accredited OR where the conditions are clean and the procedure is performed in a sterile manner.  

  If the lipoma is really this big, I am not sure that there is that much alternative to surgical excision, which could be accomplished under local anesthesia and which may be affordable.

This is an atypical presentation of a lipoma, but should be removed nonetheless.   I would recommend early excision from inside the mouth mucosa.
