I’ve had a bullhorn lip lift in 2017 and revision in 2018. What bothers me is that while the center of the mouth-base of nose distance is good , I’m bothered that the outer portions of the upper lift, left and right, are still distant from the base of the nose. It gives me down-warded, sad look. Should I opt for corner lip lit or custom revision lip lift? I've consulted different doctors, and 2 of them recommended a lip lift, but different approach.
Answer: Corner lift, scar revision It appears that you would benefit from scar revision after lip lift; dermabrasion would be a good choice. Corner of the lip lift may improve that part. In person consulation with a board certified plastic surgeon familiar with these procedures is recommended. Good luck.

Answer: Options for Lip Lift Revision: Corner Lip Lift vs. Custom Revision Deciding between a corner lip lift and a custom revision lip lift to address your concerns requires careful consideration of your aesthetic goals and the specific anatomy of your lips. Here are some insights into both procedures to help guide your decision: Corner Lip Lift: This procedure specifically targets the outer corners of the lips, elevating them to create a more uplifted and youthful appearance. It can be effective if the main concern is the downward slant of the lip corners, which contributes to a "sad" appearance. Custom Revision Lip Lift: A custom revision lip lift can be tailored to address specific areas of concern, including the outer portions of the upper lip. This approach can be beneficial if you require more comprehensive adjustments to achieve symmetry and balance across the entire lip area. Considerations: Consultation with Experienced Surgeons: It’s important to have detailed consultations with surgeons who specialize in lip surgeries. They can assess your facial structure and discuss the potential outcomes and risks of each procedure. Desired Outcome: Clearly communicate your aesthetic goals and what specifically bothers you about your current appearance. This will help the surgeon recommend the most appropriate procedure to achieve your desired look. Previous Surgeries: Given your history of prior lip lifts, it is crucial to consider the impact of additional surgery on your lip tissue and the potential for scarring or changes in sensation. Ultimately, the choice between a corner lip lift and a custom revision lip lift should be based on a thorough evaluation and discussion with a qualified plastic surgeon who understands your specific needs and can offer a personalized approach.

Answer: Recommend corner lip lift Thank you for the photos and your question. I recommend doing a corner lip lift to best address your concern. I agree that it doesn't look like you need more upper lip elevation at the cupid's bow. However, to fully assess your lip aesthetics, I would want to see how much dental show you have. Best of luck!

Answer: Mohs shaped lip lift I have stopped using the bullhorn lip lift approximately 10 years ago because of the problems you are complaining of. The technique I prefer is more of a Mohs shaped lip lift. This technique takes out wider excision laterally to pull up the corners. This is what you need now, a revision using the Mohs shaped procedure. Best Wishes, Gary Horndeski, M.D.

Answer: Consider A Nonsurgical HA Filler/Microdroplet Botox Lip Lift The use of microdroplets of Botox appropriately placed immediately above the vermilion border on the right and left sides of the upper lip can help to create a Botox "lift lift," which will secondarily help to elevate those portions of the upper lip. The use of a low viscosity, cohesive, malleable HA filler can can be threaded along the upper vermilion to further enhance the lip lift effect and further improve those sides of the upper lip. You would be wise to make sure to consult an experienced, board certified cosmetic dermatologist filler and Botox injection expert. Best of luck.
