It appears that you would benefit from scar revision after lip lift; dermabrasion would be a good choice. Corner of the lip lift may improve that part. In person consulation with a board certified plastic surgeon familiar with these procedures is recommended. Good luck.

I have stopped using the bullhorn lip lift approximately 10 years ago because of the problems you are complaining of. The technique I prefer is more of a Mohs shaped lip lift. This technique takes out wider excision laterally to pull up the corners. This is what you need now, a revision using the Mohs shaped procedure.

The use of microdroplets of Botox appropriately placed immediately above the vermilion border on the right and left sides of the upper lip can help to create a Botox "lift lift," which will secondarily help to elevate those portions of the upper lip.
The use of a low viscosity, cohesive, malleable HA filler can can be threaded along the upper vermilion to further enhance the lip lift effect and further improve those sides of the upper lip.
You would be wise to make sure to consult an experienced, board certified cosmetic dermatologist filler and Botox injection expert. Best of luck.

There is no procedure called a custom revision lip lift. Each lip lift should be customized to your situation. Lifting under the nose will not get you what you want in my opinion.
I'd perform a banner corner lip lift to end that downward "Sad" appearance. We do these frequently right in the office.

Thank you for your pictures and questions. The subnasal lip lift is a powerful technique for shortening the upper lip and everting the pink portion of the lip. But the width of that change is only as good as the width of the nose. If, for example, someone has a wide mouth compared to a thin nasal base, you are going to have a less prominent outcome, especially in the lateral corners of the mouth. There are some non-surgical options to "soften" this such as supporting the lateral corners of the mouth with fillers or injecting toxin into them muscles that work to pull down the corners of the mouth. But if you are looking for a more permanent solution, some version of a corner lift is going to be required. Hope this helps!
