What treatments would you recommend? I was considering IPL and microneedling. I was diagnosed with rosacea 3 years ago and have struggled with accepting my appearance ever since. I suffer with redness and extreme blushing - but my biggest hate is the texture of my skin..my pores are so defined and there seems to be nothing I can do, not even makeup will improve it.
Answer: Textured skin can be addressed by doing multiple treatments such chemical peels, resurfx, venus viva RF, and Clear&Brilliant Our treatments use combination therapy to improve the epidermis and dermis. We see atrophic and hypertrophic scars, as well as scars that have hypo and hyperpigmentation. Our main concern is rebuilding collagen and that is why we use a HIDEF protocol that includes combination therapy. Morpheus 8, scar subcision, fibrous release, fractional laser, CO2 laser, Thulium laser, PDL Vbeam laser, and fractional resurfacing can be used in addition to TCA Cross and chemical peels to further improve skin. If your texture concerns were in the area of rosacea, for sure I would recommend IPL, photo rejuvenation. I have great success with this combination of patient concerns improving rosacea and skin texture. This improvement is verified via a Canfield camera system, VISIA that does a pre - and post treatment analysis. Photo rejuvenation when performed correctly should really help you with your rosacea concerns. In terms of your chin texture concerns, micro needling or laser resurfacing can provide a lasting improvement. Answer: Facial Rejuv -- PRP/Microneedling, Laser (Fraxel/Halo/Co2), emeragecosmetics COSMELAN/ENLIGHTEN/EMERAGESKIN AERIFY at home peels like cosmelan, enlighten and aerify along with dermarolling (MTS roller with anteage MD ampules weekly, see linkt) should be started now to prevent further aging. For facial rejuvenation, I would suggest a combination approach of 1) skin care with products and PRP/microneedling 2) PDT light therapy few times a month with growth factors 3) laser treatments like clear and brilliant regularly and a few times a year Fraxel/halo Viva, Co2/Erbium 4) Sculptra injections a few times a year for collagen production with fillers like juvederm/restylane/belotero/radiesse/bellafill 5) thermage/ultherapy for yearly skin tightening with or without PDO or cone based instalift threads for lifting 6) botox every 2-3 months to stop movement lines and prevent lines at rest 7) deep microneedling RF like Fractora/morpheus8 for tightening of the skin 8) Vascular laser like vbeam/excel V or BBL forever young treatment every quarter (3-4 months) Best, Dr. Emer.

IPL, particularly BBL (a great brand of IPL - some systems just aren't that good) is a great treatment for rosacea. There are also often things a good medical office can help you with diet, skincare and prescription topicals. In our experience microneedling is desperately overrated by offices that find it to be a good profit center with almost no start-up or staffing costs. In rosacea it tends to increase, not decrease the root causes with no benefit. We use Halo laser which also makes small holes, but can decrease inflammation and vascularity.
