Question

The first picture is me now, the second is when i make a swallowing motion to get rid of the fat. I have bmi 20 so i dont think its just losing weight bcs when i was underweight i still had this issue. What can i do to go from the first picture to the second?

Chin liposuction can help—but based on your description, it may not fully recreate what you see in your “swallowing” photo.

When you swallow, you’re temporarily tightening the platysma and suprahyoid muscles, which lifts the floor of the mouth and sharpens the jawline. That suggests your fullness may not be just fat. In patients with a low BMI like yours, this is often a combination of submental fat + anatomy (muscle position, hyoid placement, or mild skin laxity).

Chin liposuction works best when there is true, pinchable fat under the chin and good skin elasticity. It can improve contour, but it does not tighten muscles or significantly lift deeper structures—so it may get you closer to photo #2, but not exactly the same.

If your goal is to match that sharper profile, you may need a more tailored approach:

- Liposuction for any superficial fat
- Possible submental muscle tightening (platysmaplasty) if there is banding or fullness from muscle
- Energy-based skin tightening if there is mild laxity
- In some cases, chin projection (filler or implant) to improve the angle

The key is an in-person exam to determine what’s actually causing the fullness. Liposuction alone is helpful, but not always the complete solution in thinner patients.
