I have had 2 boobs jobs my 1st one was from 32b - 32e 350cc implants under the muscle, second one 725cc in the same pocket, they have now gone very saggy and my nipple is far down. I am wondering if I get a bigger cc (850-900) over the muscle will it sort this out as I do like the faker look.
Answer: Redo of breasts augmentation redo Dear Sarah Thanks for submitting your before and after pictures and sorry for your troubles. From observing your pictures you appear to be D cup size with level 2 ptosis, large areolas, wide cleavage and severe capsule contracture. I see situation like yours often in my practice. I do not recommends to change the implants pocket location. Sub muscular placement of the implants is the best technique. I would recommend the followings: 1. Bilateral complete capsulotomy (scar release) through sub areola incision that will allow for creation of nice cleavage under direct vision and digital feel. It will also allow for easier capsulotomy, softening the breasts and creating nice slope. 2. Bilateral mini lift (donut) procedure that will create perky breasts with smaller and more attractive areolas. Due to your troubles, consult with experienced board certified plastic surgeons who do lots of redo breasts augmentation in accredited surgery center for your safety. Most important, check the before and after pictures in the photo gallery, to make sure that they are numerous, consistent and attractive with nice cleavage, perky, symmetrical and natural looking. Answer: Risks and complications increase with larger breast implants Hi,I'm afraid switching to larger implants will only create more problems for you down the line that become increasingly harder to correct. At this point, your implants are probably no longer under the muscle anyway. Your second surgery required the muscle to be cut in order to fit the larger 725cc implants. Without support from the muscle, any implant will continue to make its way down your chest. Please seek the advice of a surgeon who has experience correcting this type of problem. Answer: Over the muscle I would not put implants of that size over the muscle. Your first photo shows that you don't have enough native breast tissue to provide adequate coverage. It is fun to answer questions this way but always remember that we can't see you in 3D or see how the breasts move. You should rely much more on a personal consultation with a board certified plastic surgeon than what we recommend from these few photos.

Answer: 3rd boob job - over the muscle? Thank you for your question. I think that going over the muscle may help, especially if you don't want a lift. I usually prefer silicone implant above the muscle. As you have noticed, implants under the muscle do not keep the breasts from sagging. I recommend you get several in person opinions.
