My doctor tells me be patient it will go away and removing the sutures will cause my breast to sag. At another consult I was told that my doc might have used at larger suture that is not only visible (also through some clothing) but also very palpable and suggested having them removed stating most Docs would agree I was healed enough not to sag significantly after. Both docs are well credentialed.

During a periareolar mastopexy the skin has to be gathered to make the opening smaller by suturing the large Circle of skin to the smaller areola. This gathering cause pleats which is what you are seeing. These almost always go away with time as the implant stretches the periareolar tissue out....
I hope the NitroBid helped. It appears from the photos that change noted within on day doesn't bode well for that 1/3 of the areola that has defined itself. Good news is that the rest is ok and can the problem area can be repaired in expert hands.
