It is difficult to say from this picture an in person visit with your PS would be best for a detailed exambest wishes.

We cannot tell anything from the photo. Hard areas can either be fat necrosis or scar tissue. Darker pigment on the skin can be PIH (post inflammatory hyper pigmentation) or could be redness as a sign of infection. Fat necrosis can be painful and so can infection. Scar tissue usually is not painful but can give a pulling sensation with activity such as exercise.
