Hello! I’m a cis woman, but I feel like my prominent brow bone gives me a harsher, more masculine, or even angry appearance. I’ve consulted with a few FFS surgeons in the past. Some recommended brow bone reduction, while another suggested the difference might be minimal. I know a CT scan would provide more clarity, but based on your experience, do you think my case would likely fall under Type 1, 2, or 3? Also, would a forehead lift be a viable alternative to address my concerns? I’d really appreciate any insights you can share. Thank you in advance!
Answer
Brow bone contouring is suitable in your case. In order to know what technique is indicated (type 1 or type 3) some X rays would be necessary. If the anterior wall of your frontal sinus is thicker than the amount of setback wanted, or if there is no frontal sinus at all, type 1 technique would be indicated. If the anterior wall of the frontal sinus is thinner than the amount of setback wanted, type 3 tchnique would be indicated. Brow lift alone won't give you a satisfactory result. But brow lift must be a necessary complement to the bone work. The eyebrows must be lifted because the bone structure that gives them support from underneath will no longer be there, and also because your orbital rims will be ground on the underside (lifted) to open up the gaze.

Hello, hope you are well. Thanks for posting your questions and photos. You appear to have a relatively high/posterior hairline and a modest brow ridge. These can be tackled at the same time as a combined procedure of brow ridge reduction (probably Type I), scalp advancement (hairline lowering), and brow lift. At the same time orbital rim contouring can be done to further feminize your appearance. These are procedures I do quite frequently in the context of FFS and I think you would be pleased with the outcome. Cheers, Dr. Tower

It is my understanding that you want to improve feminine features of your forehead? If so, I do not think that contouring of the forehead would benefit you much. However, making your high forehead lower with advancement of the hairline can he considered. Hope this helps. Good luck.

No brow bone reduction or scan needed for cis female!-only an Irregular Trichophytic Forehead lift with or without hairline lowering. This is a procedure I invented 50 years ago and I’ve done more than 5000 on transgender as well as cis females. Hair grows thru the scar so there is no obvious scar. Look for a surgeon who does this procedure and can show you photos of numerous patients with the hair pulled straight back. This allows the entire brow to be adjusted and not just the outer part as with a temporal brow lift which would make you look evil!
