If the suture pulls through the ligaments, the surgeon should recognize this during the procedure as the suture will pull and not be firmly attached. The surgeon should note this has occurred and redo that region, or the procedure, since the suture will not be able to hold tension and suspend the tissues.It's difficult to make things "too tight" with the suture, because the suture is relatively thin and can break when too much tension is placed on it.So, it does intentionally cut muscle at the start as this is necessary to improve the cervicomental angle.
