My revision rhinoplasty wasx years ago. My nostrils became increasingly worse. My primary surgeon did an alar base reduction but, I think that is not what is causing the problems. I work by myself, I wear these nasal cones almost 20 hours per day. Will they help to make the nostrils more natural and symmetrical? From what I understand, there is a certain degree of plasticity in human tissue, and that is why people get dents near the nose bridge from wearing glasses. Columella is crooked too

In general I have my anesthesiologist present during nasal surgery so the pt is comfortable regardless of the level of anesthesia necessary. Having said that , over the last 35 years I've done many nasal procedures such as a tip-plasty under local anesthesia.
More pictures would be needed to fully evaluate your results. However, based on your side profile, you appear to have a great result! Your dorsal hump has been removed and your tip is no longer droopy. I would not recommend revision surgery based on your submitted photos.
