I’m 21 and have had a double chin my whole life. Even at 115lbs my neck looked exactly as it does now, and a few months ago a surgeon at an unrelated consultation told me that lipo wouldn’t do much to help that because there isn’t as much fat there as there looks like (something about a muscle being father forward than normal or something). So now I’m wondering—what exactly would help my profile?

Computer imaging is needed to help you look at the effects of any form of chin/jaw augmentation. This would be important to assess not only in the side view but in the front and oblique views as well.
I would recommend you have a Rhinoplasty and Sliding advancement Genioplasty at the same time.The Liposuction can be done later, but it probably will not be required.
