If HA filler was used, then it would be reasonable to consider dissolving it with the use of hyaluronidase, especially since filler injection results under the eyes can last quite a long time, sometimes up to several years. In the event that another filler was used or that this is swelling from lymphatic drainage problems, the so called infraorbital "puffy pillow," the addition or substitution of the use of intralesional injections of anti-inflammatory agents, such as triamcinolone and/or 5-FU would be reasonable to consider. I have dealt with a number of such situations of patients who have come to me complaining of this problem. Several treatment sessions are typically required.
