With significant neck sagging you would need a neck lift / facelift with muscle tightening (platysmaplasty). Based on these photos, it appears that a neck lift, using an already existing incision after genioplasty, would be a good solution for your problem. Suspension of the chin tissue (very difficult in the long term) or small augmentation with an implant or bone grafts, should be considered to improve chin/neck balance. In-person consultation or a teleconference with a board certified plastic surgeon, with a good reputation in face/neck lifts, is recommended.

It is very difficult to tell from the photographs presented, but a neck lift in our practice involves removal of fat deposits above and below the platysma muscle, in addition to a platysma plasty to significantly improve the jawline. At age 33, you do not need a facelift.
