hello, I have an eyelid that lies lower than the other. I know that things such as spacer grafts exist for correcting it, but I am also wondering if it is possible to do the opposite and rather than bringing the one eyelid up bring the higher eyelid lower, such as a lateral canthoplasty?

It is a fair question, and the answer depends on why the lids sit at different heights. If one lower lid is genuinely retracted (sitting too low), a spacer graft to raise it is the usual approach. Deliberately lowering the higher side to match is occasionally done, but I would be cautious, because lowering a normal lid can create scleral show or a rounded, hollow look and trades a small asymmetry for a new problem. A lateral canthoplasty changes the corner and the lid angle rather than simply dropping the lid, so it is not a like-for-like way to lower one side. Before deciding, the important thing is to work out the cause of the difference, whether it is retraction, a difference in the bony orbit, previous surgery, or just natural asymmetry, because each leads to a different solution. This really needs an in-person examination with the lids assessed for tone and position. As a rule, I prefer to lift what is low rather than lower what is normal.
