If your main goals are to remove excess skin, contour your abdomen / waistline, and improve your overall breast aesthetics then I think a circumferential tummy tuck with flank liposuction will be a great procedure. This has the added benefit of removing excess skin from your lower back and elevating your buttock (as composed to a traditional tummy tuck). Your breasts could then be addressed with either a breast lift or a breast lift with implants depending on your goals for breast size and cleavage. Lastly, your upper back does have moderate fat present but also has excess skin that will not be addressed with liposuction. A bra-line lift is a nice procedure but the tradeoff is a visible scar depending on what you are wearing. I recommend that you find a surgeon who is familiar with weight loss body contouring surgery who can talk you through the different ways these procedures can be safely staged to achieve the best outcome for your aesthetic goals.

You have lost a significant amount of weight. I would recommend a circumferential abdominoplasty that would address your abdomen, lateral thighs and buttock. A bra lift would address the excess skin of the mid back, along with a breast lift. This would be 2 procedures.
Getting an arm tuck under local with a little iv sedation for complete comfort would look amazing and be easy to undergo. Do the mid back fat lipo at the same time.

I would begin with your breasts using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. At the same time, you can undergo a tummy tuck. If you are not planning future pregnancies, I recommend abdominoplasty with mesh reinforcement. The mesh creates a more hourglass figure by compressing the abdominal wall resulting in early satiety and helps maintain a reduced weight. After you have healed from these procedures, you can consider a back lift or circumferential body.
