Hello, I have old permanent dents on both sides of my nose that I want to fix, could it be fixed through cartilage grafts with local anesthesia? I don’t wanna use fillers for the rest of my life I’m looking for a permanent solution and thanks.

Hi, Great question. I actually do full rhinoplasties, including difficult revisions with local anesthesia ... so yes it is possible. The dents that you are referring to are from a lack of cartilage support in the area. I often see these with revisions where too much cartilage was removed, or when convex cartilages were pinched inward without correction of the convexity. So the answer is yes, these can be corrected by placing grafts in the area. If fillers have worked, then a floating graft should do the trick. If on exam there is a greater lack of support, an extension from the lateral crus in the form a batten graft will be better. Hope that helps!

Any type of filling material, filler, crushed cartilage or  fascia graft can be used for the purpose of filling this groove. Fillers in the nose stay much longer than elsewhere (1 y. or longer) and may not be that bad a solution.
