As mentioned by Dr. Aldea, there are concerns with an increased risk of infection. Kidney transplant patients may also have a lower blood count as they frequently need erythropoeitin to support generation of the red blood cells by the bone marrow.
Discuss your desires first with your nephrologist and internist, IF they feel you are in optimal medical condition than you could be a reasonable candidate for the surgery with added precautions taken by your surgeon if this condition causes you significant distress.

In all liklihood you could undergo a gynecomastectomy with your kidney transplant. With your immunosuppressive therapy, you would be a slightly increased risk of a wound infection. I agree that you should discuss this with your nephrologist.
