Im 25 and I have 3 kids. I have sagging breast and im planning on getting saline tuba breast implants, High profile.The surgeon I saw said I could get the implants but i would have to get it very large ones. Im small 5ft1 and 106 pounds and a 34 a cup. Is it really possible to get the implants with out the lift? and im ok with getting a large c cup or small d cup? how many cc would i have to get? Is it even possible with out the lift?

In general, if your nipple is located at or above the crease below your breast then a lift is typically not needed. In addition to nipple position, measurements are also used to determine when to add a lift. It's important to know the breast implants do not lift the nipple to any significant degree, so you can't escape the lift by using a breast implant. Breast sagging is a spectrum and women with mild/moderate sagging can sometimes go either way. In this situation, I typically recommend breast augmentation as an initial procedure, waiting six months, and then only performing a breast lift if it is truly needed. When considering breast augmentation make sure you find a board certified plastic surgeon who will take the time to answer all of your questions and explain all of your options, so that you can decide which procedures are right for you.

Breast lifting will be necessary to achieve aesthetically pleasing results. For some patients the necessity of additional scars associated with breast lifting surgery is a “dealbreaker” ; for these patients it is better to avoid breast surgery altogether. On the other hand, many patients (If properly selected and who are doing the operations at the right time of their lives psychosocially) accept the scars associated with breast augmentation/breast lifting surgery as long as they are happy with the improvement in contour, size, and symmetry. This acceptance of the scars is the essential “trade-off” associated with many of the procedures we do in the field of plastic surgery.I hope this, and the attached link, helps.

Based on your photos I would recommend against a breast augmentation alone, particularly with a trans-umbilical approach. You have evidence of significant sagging (ptosis) and will definitely need a lift to get an optimal result. A large implant will only add unnecessary weight that will cause further tissue stretch and worsen your ptosis. Patients sometimes lose sight of the desired goal, which is a well shaped breast with fullness in the upper pole. Some patients will need a lift and the subsequent scars to get the desired shape and contour. In most patients the scars will fade with time. I would recommend that you get a second opinion from a board certified plastic surgeon. Good luck.

It just doen't make sense that if your breasts are already sagging placing a large heavy implant will cure that condition over time. Remember gravity is always working against us. As a surgeon who specializes in revisionary breast surgery, I will often see the same scenario over and over again. "My breasts were drooping, my Dr. told me that I don't need a lift, just get bigger implants" 6 months later, "my breasts are much larger than i wanted and they are even more droppy". Be careful of the surgeon who tells you that you should just put in larger implants and you don't need a lift. Many of the TUBA surgeons in this community are NOT plast surgeons, but are "facial" plastic surgeons that do breast surgery, and they have not been proprly trained in doing a skillful lift (mastopexy). The question is, how to best go from what you have to the breasts you want? Proportionate in size, nicely shaped, natural texture, symmetry between right and left, no visible scars or signs of implant(s), normal sensation, accommodating to your lifestyle.
