I have always had a very weak, recessed chin and I hate the angle from my chin to my neck. Would a platysmaplasty be a good option for me?I never see before/after pics of one on young people. If I got a chin implant would it leave me with a “witches chin” because of my deep mentolabial fold? Any opinions on non-surgical jawline contouring, chin liposuction and/or lipectomy, or kybella/cool sculpting would be great too. It doesn’t feel like fat, more like excess skin but I’m not sure.
Answer: Chin implant and platysma plasty and young underweight patient From the one very limited photograph, it appears that there is weak and recessive chin profile for which a chin implant can improve. Chin implants are manufactured in a large array of thicknesses, sizes, and shapes which is best determined at the time of the consultation and examination. A platysma plasty is not required. If there is a small amount of fat deposits in the neck, simple liposuction would be her best option. Neck liposuction and a chin implant are complementary to each other for improvement in the chin and neck area. Answer: Platysmaplasty in young, underweight patient? Hello Gypsysoul94,The platysma is a muscle that goes around the front of the neck. When young it is held together but as we age, the connection weakens and you begin to see the leading edges of that muscle. When that happens we can tighten that muscle again with a platysmaplasty to remove those bands and provide support for the neck. Because it is still in a good location when you are young is why you don't see before and afters of that procedure on young patients. As far as defining your jaw line when young this can be done with liposuction or a combination of liposuction and EmbraceRF for an even better result. Kybella helps with a double chin (off label in other locations but riskier) and cool sculpting also only helps the double chin area in larger necks. If your issue is mainly skin then EmbraceRF and possibly a chin implant would provide the most improvement. I would recommend you consult with a facial plastic surgeon to go over your options and goals.I hope this helps and good luck.

Answer: Defining the Jawline with Chin Augmentation Dear GypsySoul, You have a slightly weak chin that can be enhanced with a small chin implant and the slight fatty deposits under the chin can be addressed with sub-mental cervical liposuction that would help provide a more defined jawline. You are not a candidate for platysmaplasty. If you are uncomfortable showing a frontal photograph on RealSelf you can send one via email if you like for further expert opinion. You may have some fullness in the lower portion of the cheeks and a slight reduction of the buccal fat pad can also help accomplish more definition. Often in young ladies your age who present with a slightly weak chin, and fullness in the neck also have fullness in the lower third of the face. I perform a combination of these three procedures often in my practice however candidacy would be determined with further photos. Seek some consultations with experts in the field to discuss your options. I am not a big fan of the Kybella and have seen many patients with complications associated with its usage. A liposuction procedure of this area will improve your concerns. In the link attached below you can see many patients with these combination procedures. Best regards, Michael V. Elam, M.D.
