In this regard, you need volume (implants) and projection (the lift): Breast Augmentation w/Breast Lift.I recommend you to use microtexturized highly cohesive silicon implant ("gummy bear" implants), with high projection.Finally, to perform the breast lift I recommend a Periareolar Round Block Breast Lift ("Benelli mastopexy"), which has an unnoticed scar around the areola, and if we see (in the surgery) that we can't obtain adecuate projection just with the Benelli Lift, we have to perform a "Lollipop breast lift".Respectfully,Dr. Emmanuel Mallol Cotes.-
I appreciate your question. From your photos, it appears that you would benefit from a lift along with augmentation to accomplish your surgical goals. The size of implant best for you is dictated by your chest wall measurements. Once we determine that we can choose the profile based on what you want or need to achieve. I perform a liposuction breast lift that removes tissue from the areas you don't want such as near the armpit and the lower portion hanging near your stomach. Once I use this to shape the breast I then remove the skin to tighten the breast and create a better shape with nice cleavage. Since I perform this less invasively the recovery time is faster and drains are not necessary. The size would depend on the proportion with your body versus going for a cup size. It works very well if there are asymmetries. I can also use the removed fat as a natural breast augmentation by grafting the fat back into the breast to create more projection or into the top to create more cleavage. This procedure can also be combined with an implant if needed or wanted. It should not affect nipple sensation, mammograms, cancer risk or breast-feeding. If you gain or lose weight, the transferred fat can do the same. The best way to assess and give true advice would be an in-person exam. Thanks for your question and the photo. I think that with an implant alone the breast would not be centered and so the lift is essential. Personally, I'd be careful adding that much weight to your lift b/c the implant will pull against the repair. This is not to say that the implant won't "fit". There's no question the pocket can be made to accommodate the implant, but it might not stay in place. Your skin quality looks fine, but if you have a little ptosis you have had some stretch. Combining the implant and the mastopexy is a great operation, but it has a higher rate of revision secondary to trying to control the variables of the pocket, the implant and the lift...and then doing it on the other side and expecting it to heal the same way. Pushing the limits of implant size will result in higher risk of asymmetry. Make sure you have an experienced surgeon to help explain all of this to you. Best of Luck!
Hi, You definitely need a breast lift. However, if you haven't had any pregnancies, you may want to wait until you do. Pregnancy and breastfeeding can drastically affect the shape and volume of your breasts. The changes can also be incredibly unpredictable. That said, if you wish to move forward with surgery, consider The Horndeski Method/The Bellesoma Method to reshape and re-position your breasts without the risks/complications and extra cost associated with breast implants. Its hard to give you an exact implant size without seeing you, but a lift plus a 400cc implant could work. I will say that I tend to use a smaller implant because the heavier implants tend to stretch out the lift too soon. Get a few opinions, as surgeons vary in the way they do these operations.

Yes, I believe a lift to raise the nipple and breast up is essential, as you appear to already have grade II ptosis. Since you are a full C, a 400 cc implant will certainly make you "full". You will have more than enough volume with that measurement.
A lift possibly just a circumareola and implants will certainly be a nice improvement for you. As for the exact size, an exam would be essential.

In terms of picking the right implants to achieve your goals, generally speaking, I consider two factors when selecting implants for my patients:1. Dimensional planningand 2. Gel Implant Sizing systemDimensional planning – The measurements of your chest wall are taken. Also, the breast dimensions including the height, width, and current dimensions of each breast form the basis of dimensional planning. Based on these measurements, the implant size is recommended. This will give you a unique breast implant that is suited for your body frame. However, there are some limitations of what size we can recommend. For instance, some implants may just be too big for a narrow chest wall. Hence a physical exam is really needed to give you any meaningful advice. Gel Implant Sizing system – During the preliminary breast implant consultation, you will be provided with an option to “try on” a variety of implant shapes and sizes. You can also visualize the possible outcomes of your surgery which helps you to get that perfect size to give you the shape that you longed for. Generally speaking, patients undergoing this combination operation should understand that it is significantly more complex than breast augmentation surgery only and that it is associated with will a significantly higher risk of complications and need for additional surgery. It will be important that you educate yourself regarding the pros/cons/risks/complications associated with breast augmentation/lifting surgery.
