Nipple leaning to one side, hole in middle. Can this be fixed? (Photo)

my nipple has all of a sudden become inverted and is leaning to one side, and the right side cannot become erect. there is also a small divit in the middle and it is crusting. do i need to see a doctor? can i fix this?
