Would a second nipple inversion correction be more risky? 2 years post op, grade 3

Question

I had grade 3 inversions so got them corrected in Feb 2020. after 1 month my right nipple inverted again (grade 3 again). Unfortunately this happened at the start of covid, my surgery was unable to help and they ended up closing down since. I want to try another correction but am worried that there may be more risks since its already previously been unsuccessful - would appreciate some advice. Thanks.

Hello and thank you for your question. 
Following previous correction inverted nipple deformity, it is still possible to perform another corrective surgery but the risk of recurrence and changes in nipple sensation may be higher.
This procedure can be performed at the office under local anesthesia or can be combined with other breast procedures, such as augmentation, reduction, or breast implant removal. Inverted nipples pull inward rather than protruding outward, usually because of the underlying milk ducts being too short. During procedure, a small incision is made at the base of the nipple, underlying tethering tissues are released, nipple is lifted up and supporting stitches are placed.
I suggest that you seek an in person consultation with a Board-Certified plastic surgeon specializing in Breast Surgeries to further discuss your options, specific techniques, concerns and expectations.

I think you should be fine to undergo a second correction of your inverted nipples. It has been 2 years since your first surgery, so the tissues have healed. The fibrous attachments that tethered your nipple initially and caused the inversion were probably not all ligated. I have had to redo some inverted nipple procedures on my patients in the past and have not had any issues.
