Just turned 40, 5'7", 125. It doesn't matter how lean I get, my arms have always bigger relative to the rest of my body. Based on the photos, possible to get a good result with vaser/Renuvion alone or likely a brachioplasty will be needed? I did have three kids, gained/lost 50lbs with each, no stretch marks or loose skin, so hoping lipo/skin tightening alone will work (although not in my 20s/30s anymore)... but want to be realistic.

Hello, and thank you for your question regarding achieving optimal results with VASER/Renuvion or needing brachioplasty.

Thank you for your history and your pictures. Since you have no stretch marks, significant loose skin, or a history of weight loss greater than 100 pounds, I do not think you need a brachioplasty or arm lift. In your particular case, I believe you should achieve excellent results with VASER/Renuvion.

It will be very important for you to wear compression garments for at least 6 to 8 weeks after your arm VASER surgery. Equally important are lymphatic massages, especially after VASER/Renuvion to your arms.

There is no significant fat accumulation or skin laxity on your arms, and your skin shows no signs of stretch marks. You are an excellent candidate for achieving great results with Vaser liposuction and Renuvion J Plasma. I would not recommend arm lift surgery.

Based on your level of laxity, you should be fine with just liposuction with an energy device. The amount of excess skin is minimal and the quality appears to be good, which will work in your favor. I certainly would not perform a formal brachioplasty with the amount of excess skin that you have.

I think your arms look quite good. Could you excise a little skin and improve the minor laxity with a brachioplasty? Sure but it would leave a long scar. A mini or small T might be effective but an exam would be necessary to determine that. 
You could get a small improvement with lipo/Renuvion but it'll be subtle. Arms are high risk for contour abnormalities though so you'd have be careful. 
If you were a family member I'd probably tell you to leave them. Seek out a few consultations from board certified plastic surgeons. Lipo centers will likely encourage liposuction but leave you with track marks and indentations. I've seen a lot of revision consults for this and it's very difficult to fix.

In my mind, you are as good a candidate as there can be for this procedure. You may need two treatments to achieve the amount of tightening you desire. Also, as the skin tightens, sometimes there can be increased wrinkling in the skin. This is rare when the procedure is done appropriately, but it can happens.

I think it’s well worth exploring this procedure before committing to any procedure that puts a large scar on the arm.

Your arms look great for large weight loss.

In my experience, Vaser ultrasound liposuction removes stubborn arm fat and tightens skin without the need for an arm lift in similar patients.

An exam and consultation with a plastic surgeon who performs these procedures is recommended to confirm you are a candidate as well as discuss your options and expectations.
