Thank you for your question. Drop and fluff refers to post-operative period in which the breasts begin to descend and fills out the lower half of the breast. It may take several months for this to happen and can be encouraged with breast massage and compression. It may appear that the breast enlarges as the lower aspect of the breast fills out (at the expense of the upper portion of the breast). The changes are less with a sub-glandular pocket, but they still do settle with time.
Drop and fluff is something that is seen more strikingly with sub pectoral implants. It is related to the increased swelling of the muscle after the procedure. It absolutely does happen with sub glandular implants, but not to the degree that occurs with subpectoral implants.

In my experience, the postoperative changes (drop and fluff) seen with breast implants placed in the sub glandular position are generally less striking than breast implants placed in the sub muscular (dual plane) position. Nevertheless, I think that it is important for patients undergoing breast augmentation surgery ( regardless of positioning of breast implants) to be aware that it may take 3 to 6 months for them to see the final outcome of the procedure performed. If you have a moderate amount of breast tissue silicone implants work very well. They will not drop anymore than normal breast tissue.
You may see that happen, but those terms (which are completely inaccurate as nothing really drops or fluffs) are usually associated with the relaxation of the muscle when implants are under the muscle, as they most always are for very good reasons.
