Cardiac arrest and rhinoplasty. How common is this? Does it happen often in the US?

I read a story about a woman who traveled from the U.S to Mexico for a cheap nose job but went into cardiac arrest as soon as the doctor (who was a dentist) placed anesthesia into her spine. I heard another story about a 21 year old girl in Italy who also went into cardiac arrest during rhinoplasty. Both of them died after being in comas. How common is this and does this happen often in the U.S?
