Are cosmetic surgeons are obligated to have patient sign individual consent forms for each specific procedure that is performed in an office? I had three different procedures in one office and the practitioner only asked me to sign a consent form before one of the procedures and I would like to know if this is standard practice.? (All three procedures were fillers, however they were injected in different locations). Thank you

It would be entirely appropriate for dermal filler injections to multiple areas to be covered by a single informed consent. The purpose of an informed consent form is to provide information to the patient so she understands the purpose of the treatment, risks, benefits, and alternatives and to provide written documentation giving the injector permission to perform the treatment. Protocols differ from office to office. In our office, if the same treatment (filler, for example) is performed using different products in different areas, we provide one consent form and indicate what products are used where. This is to decrease paperwork and to reduce time. When we perform a Botox treatment and filler treatment on the same day, we prepare two consent forms, one for Botox and one for fillers. Some offices have one set of consent forms with everything listed on it and they check off the treatment that will be performed; this saves on paperwork. In our office we have our patients sign consent forms each time they perform a treatment, even if it has been performed in the past. This is a safety precaution to ensure that there are no contraindications or changes in the medical history which may preclude treatment. Please consult with a doctor for specific recommendations. Answer: Consent form Hi and thanks for your question. If you are doing different procedures like filler and Botox you need 2 consent forms but if all treatments are the same even in different parts of your body does not need a separate consent form.
