You are wise not to get the lip lift. It would drag your nostrils down and make them show even more. Lengthening a short nose is very difficult to achieve. Most results show very little if any improvement. That’s just my opinion and observation. I’m sure many plastic surgeons would disagree

Fortunately you do not appear to have a particularly short nose or a retruded columella. reducing the projection of the nose is a separate part of a rhinoplasty and would not be accomplished by the extension graft but by other maneuvers during the procedure.
