Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.Unfortunately because ~80+ % of the fat transferred will melt away within a year, most patients, like yourself, are not good candidates because they lack an adequate amount of fat to harvest. Beware, if a surgeon asks you to purposely gain weight (i.e. fat) before BBL, know that the fat you lose first as you get back to your baseline weight after surgery is that very same fat that was transferred into your buttock, hence your new buttock shrinks first, fast, and the most...so don't fall victim to this recommendation. Even those patients that had adequate amounts of fat pre-operatively, still end up seeking buttock implants after a year or so because most of the fat transferred melted away leaving them with minimal result.  If you're looking for a reliable, predictable, long-term permanent option, buttock (and hip) implants are your only solution. Size and shape can only be finally determined by an in person consultation with physical exam. That being said, an experienced buttock implant specialist can often guess very closely based upon specific photos combined with a phone consultation. Glad to help.

Dear Sarah,      Thanks for posting your pictures and your goal pictures. BBL (fat injection to the butt) is a total waste of your money and time, because the fat disappears completely or almost completely after few months or even sooner. I have seen it on many patients who came to my surgery center for butt implants surgery after their big disappointment from the BBL that was done by other surgeons.   Sarah, think about it this way. If fat injection was consistent and reliable, most women would have preferred it for breasts augmentation over breasts implants. The reality is, though, that the number one cosmetic surgery in the US is breasts augmentation with implants... BBL DOES NOT WORK !    Butt implants are your only good option, because they do not disappear and when done by experienced butt implants  expert surgeon they look very attractive and natural. If you select a surgeon who uses the sub fascial technique and oval round implants, you will get nice projection and round hips. That can not be achieved with the intra muscular technique.      I have performed butt implants surgery on over 600 patients using the sub fascial technique and oval round ultra soft silicone implants. Based on that experience and your pictures, I would recommend 430 cc or 565 cc implants according to your desire and butt measurement.    You have to realize that not all results are the same because there are different implants and different techniques. Additionally, experience, technical skills and aesthetic eye are critical for good outcome. So, do your due diligence carefully and choose your surgeon wisely. It might be worth while to travel for a good butt implant surgeon , if you can't find one locally.    Always, consult with experienced board certified plastic surgeons who do lots of butt implants surgery in accredited surgery center for your safety. Most importantly, check the before and after pictures in the photo gallery, to make sure that they are numerous, consistent and attractive with the above mentioned features and low hidden single scar.                            Best of luck,                                        Dr Widder

You seem like an excellent candidate for buttock augmentation. The size of the implant depends on the patients desired results and measurements.  A consultation is best to determine a size that best fits you.  Be sure to choose a Board Certified Plastic Surgeon who specializes in this procedure and has an extensive before and after gallery.

I perform buttock implants regularly.   Implants will give central fullness. They will not round out the hips.   Fat can round our the hips if you have donor sites. Best to be seen in person. Good luck.

you have a beautiful figure, you have a small amount of fat to be able to achieve your goals, you must increase a little more weight in order to collect the fat and be able to achieve the most curvilinear appearance in your hips.

Dear Sara Cudia,Hello and thank you for your excellent question. I have found that many patients can get GOOD results with liposuction alone, and EXCELLENT results with IDEAL Liposuction! IDEAL Lipo is a proprietary technique that we have been developing for the past 12 years. First off, all devices and instruments used are FDA-approved, unlike many fat harvest and transfer devices beying used today. These procedures are typically performed in the office, with only local anesthesia (although patients can request deeper sedation, if they so desire). We typically use Smart / Laser Liposuction or VASER (ultrasound) to reduce dense, fibrous fat, and stimulate collagen formation; Radiofrequency-based skin tightening (like Body Tite, or Thermi) to maximize skin contraction; and Ultrasound, RF, or subcision (Z Wave, Venus, Cellfina) to treat cellulite. Patients go home the day of surgery; typically do not use any narcotics postop for pain control, and are back to light exercise and work 48 hours after their procedures.
