Can I reduce just the tip and have the procedure done without anesthesia? (Photos)

Question

Is it possible to just reduce tip without breaking the nose bone? I don’t mind a slight bump towards the top, and I think the tip has always been the biggest issue. I also want to get the surgery done awake if possible and without anesthesia only numbing and wondering if this is possible? TIA

The frontal and profile photos show a broad, projected tip with relatively thick skin and a mild dorsal prominence. If the nasal bones and bridge are otherwise acceptable, tip cartilage can sometimes be reshaped, supported, and modestly deprojected without osteotomies. However, reducing the tip changes its relationship to the bridge and upper lip, so the existing bump may look more prominent afterward; skin thickness also limits how small or defined the tip can become. A very limited tip procedure can be performed under local anesthesia, sometimes with light oral or intravenous sedation, in a cooperative patient. Local numbing is still anesthesia, and being awake is not automatically safer. More extensive cartilage work may be more comfortable and controlled under general anesthesia, which also protects the airway from blood and secretions. The choice depends on the planned maneuvers, expected duration, anxiety, medical history, and the surgeon-anesthesiologist team. Ask for a precise plan showing which structures will remain untouched and why the proposed anesthesia provides adequate comfort, airway safety, and operating conditions.

Reducing and reshaping the nasal tip without breaking the nasal bones is possible with a procedure called tipplasty. This approach focuses only on refining the tip cartilages, without changing the nasal bridge.
In cases where the main concern is the tip, tipplasty can often improve shape and definition while leaving the rest of the nose unchanged.
Regarding anesthesia, performing the procedure without general anesthesia is technically possible in some settings using local anesthesia. However, we generally do not recommend this approach. 
A definitive assessment cannot be made without a detailed evaluation.

Yes, it's possible to perform tipplasty under local anesthesia. However, I personally perform it under general anesthesia, as I can perform much finer surgical maneuvers on the tip cartilage under general anesthesia. Of course, there are also surgeons who perform tipplasty under local anesthesia. You're lucky; your nasal cartilage structure and nasal soft tissue envelope quality are very good, so your chances of having a smooth operation are very high. Best wishes.
