Can anything be done about several sutures underneath the skin/scar that feel like they're about to poke through post mastopexy?

What you are experiencing is very normal during the healing phase of your surgery.  Stitches beneath the skin take months to dissolve.  Until they do, you will feel them.  Sometimes, they do feel exactly like pointy bumps that seem likely to pop through the skin.  Most will not.  The ones that do are easily removed.Your worries are based on what happened at the inverted T area.  That problem is more likely to happen because of the tightness of the repair and the blood supply to the skin.  All plastic surgeons see that problem on occasion and are able to help their patients heal well.The stitches that are not through the skin should be left alone.  Daily massage with vitamin E or a scar gel will help a bit but time is the real treatment needed.Talk to your surgeon as he or she will get you through this healing time.

 Thanks so much for your question. Unfortunately there's not a lot we can do with previously placed dissolvable sutures other than take them out or wait for them to dissolve. It's a simple matter of making a small incision and taking them out after two months time. However there isn't anything you can really do other than those two things.   Sorry that you are having difficulties.
