Thank you for the excellent question. The answer depends on 2 factors: 1) your abdominal and back skin quality and amount of adipose tissue and 2) the amount of breast ptosis (or drooping). From your photos, you are an excellent liposuction candidate. You will have a nice result without excess dropping skin on your abdomen. I cannot tell from your photos the appearance of your breasts. If they are not very ptotic, you should be a good candidate. If however, they do droop, you may be better served with a lift (mastopexy).

assuming you have no intention of losing some weight, then liposuction would do well in the abdomen, flanks and lateral thighs. I would do standard breast augmentation rather than fat grafting.
Breast fat grafting has the decided advantage of utilizing your own tissue and avoiding all of the implant related complications including rupture and capsular contracture. In addition, fat grafting by its very nature requires fat to transfer and thus liposuction is performed to harvest the fat. Thus, the patient can get improvement in the breasts but in those areas of the body that are liposuctioned as well. It is critical that the surgeon a patient selects is an expert in liposuction as well as breast augmentation.

The ideal candidate for a fat transfer procedure is both a great candidate for liposuction and the fat transfer part.generally speaking the best candidates for these procedures have tight skin, pudgy undesirable fat, or lacking volume in certain areas and don’t have other contributing variables such as excess intra-abdominal fat or muscle separation from previous pregnancies.In regards to breast fed transfer the procedure can give 1/2 to one full cup increase in long-term volume. This is about 50% to 100% pressed volume increase. Fat transfer is excellent and giving modest volume enhancement but it’s not that great at changing breast shape. If you like the shape of your breast and just want them moderately bigger than this is probably an ideal procedure for you. You look like a great liposuction candidate because you have tight skin slightly pudgy fat and no pregnancy related changes.In regards to liposuction of the abdomen I typically treat the full torso as a single anatomic unit in order to leave people with a natural fat distribution. A full torso is basically abdomen the 360 with the addition of the full back and armpits or under arm area.this generally includes liposuction circumferentially around the breast including the front bra roll with liposuction adjacent to the breast creating a clear defined breast that comes off the chest.if the back and armpit are not included patience sometimes look wedge shaped with a flaring back.The results from this procedure vary quite a bit among different providers. I would encourage you to have numerous in person consultations with local board-certified plastic surgeons who have extensive experience with this procedure. Ideally you want someone who can show you at least 20 to 50 sets of before and after pictures. I would be apprehensive of any provider showing early postoperative pictures. The results from fat transfer look far more impressive early on then they do long-term. Accurate after pictures must be taken at least three months after the procedure to represent actual results. Avoid all providers who show postoperative pictures taken in the operating room or with patients still showing surgical markings on their skin. While impressive these results mean nothing because they are drastically different than what the final results look like.Find the right provider and everything falls into place.
