The week after surgery, everything looked great. My skin was smooth, no lumps or unevenness, no fat. Starting on day 14, I started noticing what looks like fat on both sides under my chin and further down my neck. It looks funny from underneath because there isn't that puffiness directly under my chin, just on the sides. So it looks uneven. Is this swelling or permanent? I wore my chin strap for a week straight and then switched to only at night based on surgeons directions? Should I be worried?

Thank you for sharing your question and photographs.  Unfortunately without an in-person examination it is difficult to know what may be taking place.  Initially after surgery the swelling of the tissues may prevent you from seeing the irregularities in the fat or underlying muscles of your neck.  I ceertainly see your areas of concerns and it may be residual adipose, muscle laxity, or your submandibular glands but only an in-person examination can offer your a true diagnosis.  Continue to allow your results to mature and have your surgeon clearly explain the results.  Hope this helps.

You should not be concerned at the 2-3 week post op mark because you are experiencing swelling that will subside as you continue through your recovery. I suggest that you discuss your recovery expectations and best healing practices with your board certified facial plastic surgeon.

hello rebashtea During recovery, patients experience swelling, which should go down after two days, and bruising or discoloration, which will disappear in about two weeks. You should visit or contact your PS about this concern. #swelling #post-op

On the other hand, this is a surgery that really takes many months to heal.  Where you are at 18 days with swelling does not mean that this is your final result.  You will have a much better idea of this at month 4 rather than day 18.  Try to relax and let yourself heal.  It is normal to go through a number of changes as you heal.

From looking at your photos this looks like you have swollen submaxillary glands and given time this should settle down.

Although it is difficult to say what your physician did for you, one might assume that liposuction was involved.  Swelling can come and go for weeks and months after such a procedure.  It would be difficult to make judgements about the final result at this early time.  I tell my patients to wear their chin strap both at night, and all times during the day when the are not in public.  This can assist with swelling when the device is off.  It is also helpful to ask your treating physician about your issues if you have concerns.  They are willing to speak with you, and allay your fears.
